As you would like it to appear in concert programs.
If you would like the Academy student's name to be on the email list for Academy updates add the student's email.
If you would NOT like the Academy student to get email updates then just put the main parent email.
Instrument the student will be playing at Academy
ONLY fill out Instrument #2 if student will be registering for 2 ensembles.
This parent will be the Director of Education's main contact for the Academy.
Materials for Academy or other related information will be mailed to the address put down for the main parent.
If you put down a second parent email this parent will receive regular Academy emails from the Director of Education.
If you don't want the second parent to receive emails DO NOT put an email for this parent.
Only add an address if it is different from the First Parent's Address.
